Registered EMTS and Paramedics - Register Online
We have temporarily closed our COVID 19 online registration site. We expect to reopen the site soon. Please do not call JRMC to register for the COVID 19 vaccine or to schedule a COVID 19 vaccine appointment.
Moderna COVID-19 Vaccinations for EMTS and Paramedics.
Registration for Saturday January 2 is full.
Please continue your registration and you will be notified of the next available date in January to receive the vaccination.
Once you register online, someone will call you to confirm your registration and set up a time.
Vaccines will be given by appointment only. No walk ups will be accepted.
Additional Instructions:
- Please choose “Other” for testing location.
- If the patient is a minor, the parent must register the minor and electronically sign the parent consent form.
- A telemedicine visit is required prior to testing. You will be contacted.
Registration for Old Bridge testing on June 1st is now closed.

If you are a new patient at JRMC, you MUST print and complete ALL forms in the packet below and call 732-376-9333 for an appointment.
